Hai guys. 😁
Today I would like to teach about Acute Coronary Syndrome(ACS) which is a common topic to be mastered by medical students.❤️
I will try to make you understand ACS in a simpler way.😚
Hope all of you will benefit from this posting.
⚠️ACS can be divided into 3 :
1️⃣Unstable angina
2️⃣Non ST elevation myocardial infarction(NSTEMI)
3️⃣ST elevation myocardial infarction(STEMI)
🧰Mechanism of ACS :
⛑️rupture or erosion of fibrous cap of a coronary artery plaque
☠️Unstable angina can be differentiated from NSTEMI by :
1️⃣ECG
2️⃣Cardiac biomarkers
❣️The best cardiac biomarker is :
Troponin T and I
🤎The best cardiac biomarker after Troponin is :
CK-MB
💊Principle of STEMI management :
🤕pain relief
🗑️establishing early reperfusion
📛treatment of early complications
2 types of reperfusion strategy :
📌Percutaneous coronary intervention(PCI)
🧼fibrinolysis/thrombolysis
⚠️⚠️⚠️Indication of of fibrinolytic therapy :
should only be given to patients with STEMI only!!!!!!!!!!!💢😡
👇Inferior myocardial infarction(MI), we must do right sided ECG to look for :
lead V4R ✌️4️⃣ for right ventricular infarction
🩺Treatment of right ventricular infarction :
💦optimisation of IV fluids(key therapy to correct hypotension)
💘inotropes
💊2 drugs contraindicated in right ventricular infarction :
1️⃣nitrates
2️⃣diuretics
💊💊💊For the treament of NSTEMI, the mnemonic that I use is :
"MONARCH BASIC"
"MONARCH"
"BASIC"
