Facial Nerve Palsy Made Easy

Hai guys.

Today I would like to share about facial nerve palsy as it is quite common in stroke patient.

🧟Facial nerve palsy


1.Lower motor neuron lesion(LMNL)

✅ IPSILATERAL LESION
✅ No Forehead Sparring

2.Upper motor neuron lesion(UMNL)

✅ CONTRALATERAL LESION
✅ FOREHEAD SPARRING

3.Causes

✅ UMNL: stroke, tumour
✅ LMNL: Bell's palsy, otitis media, skull fracture, Herpes zoster(Ramsay Hunt syndrome)

4.Physical examination

✅ Hearing: louder than usual (hyperacusis), inspect external auditory meatus(Herpes zoster lesion)

✅Sensory: change of taste( anterior 2/3 of tongue)

✅Face: 

-forehead wrinkles
-loss of nasolabial fold
-drop of angle of mouth

✅Facial movement:

1.Frontalis: raise eyebrows
2.Orbicular occuli: close eyes and don't let the examiner open them
3.Orbicularis oculi: puffed cheek
4.Zygomaticus major: big smile
5.Orbicularis oris&buccinator: purse lips(try to whistle)
